Complex Regional Pain Syndrome: Ketamine’s Pharmacological Effects and Modern Treatment Applications

Abstract

Complex regional pain syndrome (CRPS) is characterized as a severely debilitating and painful disorder that is described by the notable functional impairment and the chronic disabling pain which is disproportionate to its inciting event. There are many theories exists regarding the pathophysiology of CRPS, however nothing precise has been elucidated. Central sensitization is  from proliferation of the N-methyl-D-aspartate receptor (NMDAR) may lead to disabling pain syndrome. The NMDA antagonist, Ketamine appears to play a unique role in managing the pain involved with this syndrome by reversing the central sensitization and providing analgesic effects. Ketamine's structure and metabolites demonstrate promise for controlling the debilitating pain that patients suffer with this disease. Long-term use of ketamine and its efficacy is questionable, therefore it is recommended that ketamine infusions be used with caution and reserved for refractory cases of CRPS.
